We’re on a Mission
CircleLink Health® is a company of passionate clinicians, technologists and business people tackling the $600B problem of preventable chronic condition complications. Our mission is to accelerate the shift to preventative care (from status quo reactive care) through our world-class preventative care platform.
From the start, we’ve believed patients benefit most from personalized and provider-led follow-up between visits. We launched our diabetes module in 2010 (intelligent texting on behalf or Dr.s), and have since launched protocols in most major chronic conditions. Starting in 2015, we folded this expertise into our award-winning chronic and behavioral condition coaching platform, manned by our amazing registered nurse network. This platform drives improved health and significant new provider profits.
Our commitment to excellence sets us apart
100% Registered Nurses

Proven Results
Our award-winning platform has helped thousands of patients with chronic disease care management, behavioral health management and more. We’ve driven millions of new revenue dollars to provider organizations.
Zero Staff Burden
We do the work to integrate into your existing workflow, find and enroll eligible patients, create care plans, deliver audit-grade billing reports, and provide medicare chronic care service logs.
